The Senior Wellness Program at Minor & James is dedicated to providing the best care possible for residents who live in the long-term care and retirement home setting. Our program is staffed by several physicians and nurse practitioners who are trained in the care of older individuals.
We try to cover the spectrum of care from the outpatient setting, to inpatient hospital care, and ultimately to the long-term care setting where much of our expertise resides, if such becomes necessary for rehabilitation or for the provision of long term skilled care. The program has been in existence for over 10 years and has persisted through times of change within the health care arena. Some specifics of the care provided include comprehensive functional and cognitive assessments, family conferences, and coordination of care with other physicians and various facility staff.
