If you are a new patient to the clinic, or an established patient with address or insurance changes, please plan to arrive 30 minutes prior to your appointment to allow adequate time for updating your patient registration information. Please bring your insurance identification card, Labor & Industry claim number, or current Medicaid (DSHS) coupon, if appropriate. If your visit pertains to a motor vehicle accident, please bring any necessary motor vehicle accident claim information.
